About the role
The Case Manager, Team Leader provides leadership and direction for our team of Case Managers who work across both metropolitan and regional locations in Victoria and Tasmania. They are responsible for driving business growth, marketing our services to the community and promoting excellence in the provision of person centred care. The role requires a proactive management style with the ability to prioritise package referrals (an area of growth for Calvary Community Care), working with our Care Advisor team to engage with potential new clients and sell Calvary service offerings.
As the Team Leader, you will drive best practice Case Management for our customers through coaching and mentoring of your team to deliver on KPI’s and exceptional customer service delivery. The role will work with Case Managers to review, evaluate and modify care plans. You will ensure the promotion of independence and promote the achievement of customer goals and establish positive working relationships with other professionals and agencies through participating in relevant forums and meetings.

We are looking for candidates with the following:
- A resilient leadership style with a tertiary qualification and recent experience in the sector
- Strong people leadership and management skills with the capability to build a strong team
- Knowledge of the scope of practice of a Case Manager in a community setting
- Dynamic communication skills and the ability to build new business and promote the Calvary brand
- An ability to build and maintain strong partnerships with stakeholders
- Passionate about identifying customer needs and areas of opportunity
- IT proficiency with strong MS Office, databases and system skills
